Currently I am using Qmail MTA with most plugins and a very customised SMTP server which I have re-written most of which is capable of executing code before passing the mail to qmail-queue. If it does end up successful it would obviously be usable publicly. Oh and also if necessary I can run code on Windows systems and get Qmail to send the relevant messages to a subdomain to an SMTP server running on the Windows box as a last resort, kind of like the already used mail2news/news2mail system I know some of you already use here.
