Question Regarding:reconstruction of deviated septum and removal of

Viewers Question
I had surgery last August to repair a devated septum and the removal of sub
turbinates. After the surgery I was in extreme pain. After a few weeks, I
discovered that I had a huge hole in my septum. My doctor at that time told
me that it was nothing to worry about and that it was a minor complication
from the surgery. I saught out another Doctor, who told me that it was not
a minor complication but a rather rare complication, and that I was missing
more then 3/4 of my septum. This new doctor performed a 5 hour medical
procedure to close the perforation by surgically using a fascia graft from
the scalp. This procudure also failed. I am very frustrated with the first
doctor that performed the 1st surgery and its out come, and how I was
treated. I a sad that the second surgery was not a success. I am at a loss
as what to do next! just live with the hole in my nose and the constant
dryness and slight bloody noses...or seek out yet another specialest. Can
you try and lead me in the right direction? Thank You, Nancy W.

Nasal obstruction is usually caused by a deviated or crocked septum or
enlarged tissues (turbinates) within the nose.

A septoplasty is an operation to correct a deformity of the partition (the
septum) between the two sides of the nose.

A turbinectomy is the surgical removal of an abnormally enlarged turbinate
from insdie the nose to improve breathing. 

A septal perforation is when the flaps lining the bone or cartilage of the
septum fail to heal properely. Predisposing factors include the use of
over-the-counter nasal sprays or cocaine. I see septal perfortations
frequently in my practice, and I believe that they occur in several percent
of patients undergoing surgery on the nose. In almost every situation, the
perforation only causes minor problems such as crusting, dryness, or
bleeding. In some situations, the problems are more difficult. 

The best type of procedure to correct the problems is to use a graft from
the scalp to reconstruct the septum. At this time, new tissue is brought in
from the side of the nose to help it heal. The other options include
buttons, but in my experience most patients do not like the way they feel.

Obviously I cannot recommend what should be done in your particular case,
however, I would suggest that you talk with your surgeon regarding other
options.
