I curious how could this functionality be implemented? For example, an 
XML editor or DocBook editor that always enforce schema validation on 
every action performed by users.

I'm sorry but this isn't supported and I don't know of any implementation
that can provide you with this functionality.

Maybe JAXB might help you out here?
