The stats are that close to 80% of the e-mail traffic that is on the net is spam. Although my ISP uses "loose" settings and I get a lot of spam, I still loose some e-mails that are "real e-mail".
One week my ISP had issues and dropped the spam filter and I had roughly 100 spam e-mails a day. I am not trying to defend the ISPs poor quality implementation, but it is a necessity to run spam filters, and unfortunately they are not perfect.

This contributes to missing posts from reflectors, general email, etc.? In addition, some email gets delayed for days or weeks and then magically appears.? Not sure how many ISPs have this problem.
