Here is another reference to Dr. Henry Stockwell of the Canal Zone, 
evidently an "amateur" entemologist, referred to in the text as a 
"medical doctor."  Scroll to the bottom of the PDF file for the 
article.  There is a picture of Dr. Stockwell.

And finally, here is Dr. Henry Stockwell, 48 years of experience, 
pediatrician in NC.  Referenced is a research paper on "boy in the Canal 
Zone."  I'm betting this is your man.
