RE: Please help with title

The hebrew letters following "sefer" are "mem-h.et- gerashayim-yod" and
"mem-samekh-yod" and the middle name is "mem-nun-yod-nun-yod"
 
I couldn't find a record on OCLC for this one (only some mss. for different
ones from JNUL only in Hebrew.

Any idea how to romanize the îðéðé part of the name? the full name is øôàì
