I don't know about all these things so my suggestion may be a bit off. I _have_ done PDF markup in two ways: 1. Write text "on top of" the PDF file. One can do this with flpsed which is short for "fltk PostScript Editor". 2. Fill up PDF forms by extracting the FDF data. One can do this with pdftk.
