Pam,
I've been having this problem for a long, long time with the
descendant book printed in RTF.
I've never been sure if it was Legacy or MS word!
I manually remove the blank pages, which also removes the
page numbers and printing date.  Then add the page numbers 
back and manually add the date to the cover page only.
After all those manual fixes I do the table of contents and the
name index.
Then DOUBLE CHECK the table of contents and name index.
I've sent reports and usr files to Legacy, so far I've heard
nothing.
Half of me is sorry that you are having the problem too, as it is
a lot of work to fix, and half of me is glad I'm not alone!
I have about 14 reports I have to do and very soon!
