Ok, I'm now 100% sure this is a Mozilla bug, specifically Bug#136749.
It's also not that critical because merely collapsing and re-expanding
the hierarchy makes the subfolders show up again (at least in recent
Mozillas).
However, for completeness here's how to reliably reproduce it...

Manually create a fresh folder hierarchy, as follows

   mkdir Maildir; cd Maildir
   for i in A B C A B.a B.b B.c C.a C.b C.c C.c.1 C.c.2 C.c.3; do
      mkdir $i $i/new $i/cur $i/tmp
   done
Start Bincimap

Start a fresh instance of Mozilla or Thunderbird, with no existing
configuration for this mail server.  Configure an account for it.
Log in.  You should see INBOX and nothing else.

Right-click on INBOX and choose "Subscribe".

You should see INBOX, unexpanded.  You can now expand it all the
way down the entire hierarchy... so far so good!!!
Click the "Refresh" button.  Boom.  A, B, and C now no longer have
subfolders.  Collapsing and re-expanding the hierarchy deterministically
makes the subfolders show up again, so it's not that critical, merely
annoying.
