libxml2 only allows numeric entity references in this range:
*  Char ::= #x9 | #xA | #xD | [#x20-#xD7FF] | [#xE000-#xFFFD]
* | [#x10000-#x10FFFF]
Racket doesn't fully respect that, but both disallow 0. I think we
should match libxml2 and tighten the contract.
