Hello,
We disabled some of the fields on the incident form and saved as a customized 
Incident.  The customized Incident is sealed.
Now we are trying to remove this customization form and roll back to the 
original Incident so that we can create new templates.
When try to delete the customization form, we are getting a dependency warning:

Here are the dependencies.

How can we remove this customization and put  back the original Incident form?
Can someone help please?
