document where to discuss OS and compiler issues and report problems

It would be useful to document where OS and compiler issues (as well as issues 
in any other third party software that stdcxx depends on) should be discussed, 
questions raised, and where problem in such software should be reported. An 
easily editable Wiki page with this information seems like the best place to 
put it.
