[Trac] Trac Limiting Attachments well below the listed max

Running trac 11.6 via CGI.  For some reason, I can upload a 941 byte png as an
attachment to a ticket, yet a 15 KB png fails.  File size limit is 256kb, set to
262144 bytes in the trac.ini.  Log posted to http://paste.lisp.org/+22KL.  I've
been using trac for a couple weeks.  Any thoughts on where to look to figure
this out would be greatly appreciated.
