Does the KVM use Apple style connectors (large D, 2 rows of pins) or PC
style SVGA connectors (small D, 3 rows of pins, slightly staggered) on
*both* sides? Are you having to convert between Apple/PC at any point? This
could be the problem.

It sort of sounds like 2 things may be faulty: 1) The monitor is expecting
extra information via sense pins and not getting it/getting muddled, 2) the
power output from the KVM has degradated the signal and the monitor is not
detecting it... Is it an electrical switch or is it a manual switch?
Sometimes manual ones spike the power and cause the monitor/keyboard to have
a fit.

I have a manual 2 way "data switch" style KVM at home with an old 17" compaq
deskpro monitor. It happily changes between two computers, though I have 3,
so I need to acquire another switch (mental note to self,,,) The only
problem I have it that the monitor will *sometimes* it get's a little
confused and will act like there's no signal... I usually have to turn it
off and on again to get past that. Switching between the computers has no
effect in this scenario...

Hope that helps,

Matt
