Are you trying to connect to the AV card or the built-in (HDI45) video?
Anway, I recall that the Basic monitor, although Apple-branded, was a VGA
monitor of 3rd-party manufacture. It needed an adaptor (part # 936-0112) to
work with a Mac port (either kind). That didn't have switches, (as there is
only 1 resolution -- 640 x 480 @ 60 Hz.) But apparently the 3rd party
adaptor you're is using has switches, & it might be that you have them in
the wrong position, especially in regard to the kind of sync. The Apple
Basic Monitor needs separate (horizontal & vertical) sync.

You might find you prefer to use the Performa monitor, mediocre as they
were, to the Apple Basic when you get it running....39 mm dot pitch,
forsooth!
