Another reason this error could be seen on Mac OS X is that if the helperTool is not installed and the web server attempts to publish to ports below 1024. I know you mentioned using ports 8080 and 8081 and 8081 but also make sure that the TLS/HTTPS port is either disabled or above 1024 to avoid needing the helperTool. Also, don't utilize the 'start web server at launch' database settings but try using the actual command or menu item to start the web server instead.
