To close this out I'm going to finger server 15.4 as the problem here.
Looking back at my notes I see I didn't put in the upgrade previously but
did it this weekend. I updated my development machine to 15.4 a while ago
but not this server.

When I rolled back to v15.3 everything went back to running as expected.
There was at least one other thread talking about 15.4 instability.
Personally I'm avoiding it until 15.5.
