1. Go to the Device Manager.  Delete any entry for the USB camera, or
"Unknown USB device", etc.
2. Uninstall the driver software.
3. Unplug the camera.
4. Reboot.
5. Install the driver software first.  Follow all steps carefully. 
Don't plug in the camera until instructed to do so.
