Ah, i've been able to reproduce this.
It's only from Flash applets and only certain ones.
It seems to be a new option in the latest version of the Flash plugin.
If your right click it and pick "Settings..." from the menu, it shows
up in one of the settings.  I have no idea what Macromedia's
intentions are but the default is disabled for all sites.

That said, a bare install of Firefox doesn't have Flash installed in it.
