Take a look on:

Your card is likely a Voodoo 2 or Voodoo card.  Look for additional
markings on the GPU chip or the otherside of the card itself.
3DFX never supplied universal drivers do you need the driver for the
correct Voodoo series for it to work.  
NOTE: For these drivers, you need to use the installer program to
install the drivers and not the Device Manager.
