That file IS in the distributions; I have copies in these versions:
a2ps-4.12d a2ps-4.12f a2ps-4.12g a2ps-4.12i a2ps-4.12j a2ps-4.13
a2ps-4.13b
If a2ps is properly installed in the default location (prefix=/usr/local),
then you should have something like this:
% ls -l /usr/local/share/a2ps/ppd/level1.ppd
-rw-r--r-- 1 beebe staff 1892 Feb 11 2000 /usr/local/share/a2ps/ppd/level1.ppd
