More cast cleanups

I cleaned up the casts word-plugins to use C++ style casts. I also did a little bit of format cleanup.

I built and tested with some documents all of the ones I am submitting. I cleanup the rest of the plugins, but
I have been having some problems getting the make system to try and make certain plugins. Hopefully I will
have this solved soon.

The cleanup of OpenWriter is based on the cleanup work that got committed earlier today.

Hopefully more cleanups to follow.
