ToC and page number questions

I have two questions here

1) I know that automatic ToC has not been implemented yet but how are people doing manual ToCs? I want the paragraph number and the title on the left of the page, the page number on the right and perhaps a line of dots connecting them. Is there any easy way of doing this or are people just doing it manually and just dealing with the wrapping etc.?

2) How do I get two sets page numbers, one for the introductry material in lower case roman and then reset to page one and switch to normal arabic?
