This article walk you step by step...this article assume that you have full [DBA]access to the SQL Server:

If you don't have full access to the SQL Server [you can only connect to the SQL Server e.g. link table] then you can do the following:
- Create an ODBC to connect to SQL Server
- Create an Append Query [to append to the SQL Server's table]
When transfering data you MUST consider the data type equivalents for both database; check this article for that:
