We use WM_GETOBJECT, SPI_GETSCREENREADER flag, and additional low level system info. It is mostly an app development problem, not an AT problem, although to find a solution we would definitely need both sides implementing the handshake mechanism. So that's why a forum like this would be the appropriate place to come to an agreement.

I think some sort of handshake makes a ton of sense.  We have event mechanisms in IA2 now.  Since (as I understand it) MSAA/IA2 is essentially synchronous, having the app receive a "I'm an AT and will be asking you for stuff" message, and then having the app emit some event that says "I'm ready for your questions" seems like a reasonable way to provide asynchrony for this. 

If we were to introduce this pattern, how quickly could Windows AT adopt it I wonder...?
