If an accessible whose backing control has been destroyed still has a
ref count is still greater than one due to an AT and/or oleacc still
holding references what response should the server provide to the AT?  I
assume the state should be set to STATE_SYSTEM_UNAVAILABLE, but for
other than get_accState what failure code should be returned when any of
the IAccessible methods are called?  S_FALSE?  E_FAIL,
CO_E_DISCONNECTED?  Something else?  In this case, once the AT and/or
oleacc do the final Release the object's destructor will be called.
