There was a discussion on the Orca mailing list recently of an attempt to implement structural navigation for Libreoffice in Orca. This was put on hold due to Libreoffice accessibility bugs - it might be a good idea for someone involved in Libreoffice to liaise with the Orca community to make sure those bugs are properly identified and worked on.
