Hello, Railways does allow changing of names upto 24 hours prior to travel.
So you can easily change your escort by visiting the station master
concerned.

Hi, I have successfully booked 3 tickets so far. Haven't faced such issue.
There may be a network issue with your mobile carrier. I have a
vodafone connection and I receive the OTP as soon as I hit the
'verrify', and the 'book now' buttons. You must be very careful about
the exact details on the card. For example, the railway officials
added 'Mr' in front of my name and I must type it exactly as it
appears. You must also type the ID number along with the alphabets
signifying your railway zone and the allotted number your date of
birth comes after the ID number and then comes the expiry date of the
card. the secret code is only the OTP you receive on your mobile.
before that you must also choose the appropriate category i.e.
handicapped/escort/general. for the escort ID card details are
automatically filled and you must fill up his/her id card details
extra i.e. voters id its number etc. After entering the OTP when you
hit the 'go'
button, you get all the details regarding concessional fair
availability etc. Thereafter the process is same as for other
transactions.
