It is slight differ from office to office. Some offices like Pune have there
own application format and some offices demand application on plain paper
with your signature.
You have to submit self attested xerox copy of all the documents and present
the original for verification if demanded by the official.

Hello Friends,
many of the list members have already made railway ID card, so please
do let me know are these the docs which we need do submit in
commercial office in railway station.
1. copy of railway concession certificate in new format
2. copy of medical board certificate showing 100% blindness
3. copy of 10th class certificate
4. residence proof like Election ID or Aadhar card,
5. 3 passport size photograph & on top application signed by you
Here I would like to know we need to submit
1. original or xerox copy of railway concession form
2. application form down loaded from online (format of unique id card)
Please clarify my doubt.
