1. Coverage to persons with disabilities between 18 years and 65 years with annual family income of less than Rs. 3,00,000/- per annum. 2. Same premium across age band 3. Health Insurance cover up to Rs. 2,00,000/- per annum as family floater 4. No pre-insurance medical tests 5. Family Definition: Self, Spouse & up to 2 dependent children * Documents Required: 1. Duly filled and Signed Proposal Form 2. Passport size photograph of each family member and copy of any Government ID Proof i.e. Aadhar Card, Voter ID, Driving Licence etc. 3. Proposer required to pay only 10% of the total premium of Rs. 3100/- + Service tax under the scheme i.e. Rs. 310+ Service tax 4. Disability Certificate 5. Bank details of proposer i.e account number, bank name, IFSC code etc with copy of cancelled cheque if available.
