Well, the card number is unique. No 2 persons shall have the same UID number

Tips for filling disability ID online form:

1.        Form has bilingual fields in English and Hindi for personal info
and address. Typing in Hindi is tricky. It is phonetic typing. It shows 
suggestions list if you press backspace for editing.
2.        There is a check box for permanent address as 'same as above' as
present/correspondence.  However it is placed at the bottom so high chances 
that we will miss it.
3.        There is an option if you already have disability certificate to
upload.  So a radio button option yes or no.
4.        You can update your details after submission by logging in with
provided enrolment no and DOB.  You get enrollment number after successful 
registration.
5.        Before final form submission and for log in with enrollment
number there are image and audio option CAPTCHA.
6.        Please keep following info handy.
a.        Your online disability certificate no. and date of issue
b.        Your identity proof info
c.        Your aadhar number
d.        Percentage of disability, category, etc.
e.        All relevant personal and address details
7.        Please make sure you have following files ready for upload
a.        Signature / Thumb / Other Print
Only jpeg, jpg, gif and png image with size 3 KB to 100 KB allowed)
b.        Photo *
(Only jpeg, jpg, gif and png image with size 15 KB to 200 KB allowed)
c.        Upload Proof of Correspondence Address*
(Only jpeg, jpg and png with size 10 KB to 800 KB allowed)
d.        Caste Certificate

e.        Nature of Document for Address Proof* : select option from voter
id, aadhar card, ration card, etc.

Online Application Guide

Step    1: Person with Disability will click on register link to register
with UDID Web Portal.
Step    2: Using credentials PwD logs in to system and click “Apply online
for Disability Certificate. Reads instructions and fills up online application.
Step    3: Upload color passport photo and other requisite documents like
Income Proof, Identity Proof and SC/ST/OBC proof as required.
Step    4: Submits data to CMO Office/Medical Authority.
Step    5: CMO Office/Medical Authority verifies data.
Step    6: CMO Office/Medical Authority assigns the concerned specialist(s)
for assessment.
Step    7: Specialist Doctor assesses disability of PwD and gives opinion
on disability.
Step    8: Medical Board reviews the case and assign disability percentage.
CMO Office prepares Disability Certificate and generates UDID and Disability 
Certificate.
Step    9: UDID datasheet goes for UDID Card printing and Card dispatched
to PwD.
