Hi Depending on the address in your Adhar card, you will have to go to the Civil hospital falling in the purview of it. Find out the day in which they issue medical certificate including for rail concession. Usually it is on Wednesday. Get yourself a certificate of blindness and also in addition request for Rail concession certificate. They will issue one and this has to be send to the nearest Division manager of railways.
