I am little concerned about the lengthy form which is attached with the invitation. It has asked for so many fields and some of them probably do not look to be relevant to the quiz process. For example: 1. Father's/Husband's name - This probably has nothing to do with determining if the person is disabled or something. You might be leaving some people out with this question as well. 2. There are fields to fill up DOB and Age and both are mandatory as I remember. This looks like duplication in all means. 3. % of disability - Many people might not be having the data (I don't have it and many people from the list whom I have talked to don't have either). If you are setting any qualification bar or any extra facility based on this data kindly indicate that as well. 4. Disability Certificate - Recently I have observed many forms asking for Disability certificate. When it comes to a course enrollment, scholarship or similar activity it is quite understandable but probably it can be skipped for co-curricular activities like this. I am raising this point because the nation is in a lockdown for 7 months now and many late disabled people might not have built up the courage to visit a city hospital for the certificate amid corona risk. 5. Photo - I have participated in numerous quizzes in my short life. I was never asked to upload my photo in any case. Also the quiz registration form just reminded me of the form I filled for my PAN Card :) I hope I have made some sense with my observations and probably will lead the organizer panel to give a thought.
