You are quite right, there are many accessibility regressions in some
last versions of Unigram.
However, I'm in touch with it's dev and also contributed some fixes on
it's Github,
May be he'll murge them in next release.
Meanwhile, 1 thing can be slightly helpful for you if you use NVDA:
You can use Unigram Plus NVDA addon and it will enhance somethings.

The keys are not behaving as expected for example the escape key

in channels, When the files are uploaded combined as a folder, the
name of the particular file is not announced while downloading them
and instead announced just “link”.
There are no option to create groups/channels and if there are, not
accessible with any screen-readers.
I am not worried if the apps are not accessible as there are many but,
concerned because, this app advertises itself being accessible to the
blind and doesn’t care about accessibility any more.
