Is it a disability certificate or UDID card? For lost UDID card first, one should file F.I. r. with police station and with that copy person can reapply for UDID card by 'Lost card' link on UDID website.
For lost disability certificate : There are two type of disability certificate. one is on central level and other is on state level. One can download national level disability certificate from UDID card website. For state level disability certificate, one should visit civil hospital for reapplication.
