3. type your date of birth in dd/mm/yyyy format.
4. solve the math captcha.
5. after successfull login you will land on web page where you can
download your disability certificate and UDID as well.

You can take print of digital UDID card as color card and use it
untill you get original physical copy you do not have to do any
thing they will post it as it will be ready i got my physical card
after 1.5 year
