If an applicant applies using a valid UDID card, no separate photograph
upload is required. The applicant’s photograph and personal details are

Indian Railways does not issue a separate physical concession card.

For Manikanta Polavarapu (Doctor’s Details):
In the UDID-based application process, applicants usually do not need
to manually enter the doctor’s name or registration number, as these
details are auto-filled from the UDID records.
Generally, the doctor’s name is mentioned on the Disability Certificate
or Diagnosis Sheet. In many cases, the doctor’s registration number is also
mentioned on these documents.
However, this is not uniform for all applicants—some Disability
Certificates include the doctor’s registration number, while others do not.
This variation depends on the issuing hospital.
If an applicant applies without using UDID, then a physical Railway
Concession Certificate is required, and in such cases the doctor’s full
name and registration number must be clearly mentioned on the doctor’s seal.
For Yasir Jan (Eligibility Criteria):
Eligibility for railway concession varies depending on the type and
extent of disability, as per Indian Railways rules.
For example, Persons with Visual Impairment are generally eligible for
concession when the disability is 90% or above.
Other disability categories (such as Orthopaedically Handicapped,
Paraplegic, etc.) have different eligibility criteria, based on the nature
and severity of the disability.

It ask for 16 digit UDID they have not count the state code prefix
of it. so all must enter 2 + 16 18 digit UDID card. My concession is valid
till october, hence I did not saved and submitted it.
