Hi,
Try using the two-finger double-tap gesture anywhere on the call screen to answer or end calls while TalkBack is running. This works for both regular phone calls and WhatsApp calls, so you don't necessarily need to activate the Accept or Decline buttons directly. Hopefully that helps.
