You can use the Speech History NVDA add-on to copy the text spoken by NVDA, which can help when it is not automatic. This tool is also useful for other tasks.
Whenever NVDA speaks the CAPTCHA, you can press F12 (the default hotkey) to copy the content to your clipboard. You can also customize this hotkey in the settings.
