Yes, it get struck when the file download dialog comes and it attempts to
get the file information. I re uploaded it but it still happening the same.
I think there is some technical problem with the dropbox site. some times it
comes without problem. so, it is uncertain. if you cannot get the save as
dialog within few seconds, don't wait further and retry again from the link.
I have tested it and some times it works. I have already sent it privately
to 2 persons who requested it. if any other person is in urgent need and
cannot download, drop a private mail to me requesting for the same by mail.

I have uploaded the excel file and you can download it from the link below.
you can either fill the form in the soft copy and print and sign and submit
or get the printout of the blank form and get it filled manually. before
printing, make sure to replace the name of your institution with Tumkur
University and your name and designation with mine. the file contains two
pages of the format available in 2 separate sheets. one is the tax statement
and another is the salary abstract. one more thing, this is not a statutory
format hence you can make suitable small modifications.
