While most of the points that you listed are generally valid, every department has its own admission procedure. So, the process adopted by say Computer Science at Stanford might be totally different from English at Stanford. Further, admission procedures tend to vary substantially from university to university. @Aziz Your best bet, as pointed out by Sathiyaprakash, is to refer to the admission website of the concerned department of the concerned university.
