On your second form, I would use something like this:

<input type=checkbox name="somecheckbox" value="ON" <%
If rs("somecheckbox") = "ON" then
    response.write "CHECKED"
End If
%>>

It checks the database, and if you stored the data from the previous
checkbox, then it writes "CHECKED" on the HTML code so that it will be
checked on the second form.
