I am doing exactly the same as you do but it does not work for me.
After looking in greater detail at the file generated by GraphicConverter,
found that the file type was not the right one probably due to my version of
GC. I switched to the Iconbuilder plug-in for Photoshop and I am now getting
a favicon that properly shows up.
