The code that does the serving didn't really change, but A4D_Init and A4D_DebugInit changed quite a bit. If you didn't integrate that code, you won't get very far. In addition, A4D_DebugInit must be called _before_ A4D_Init.
If you are getting 404 on every request, it could also be that ExtensionsMap.ini is either damaged or missing.

In particular, you also must ensure that A4D_HouseKeeper is called.
