If you're not afraid of working on Windows, RoboHelp can do this kind
of stuff.
We settled on this years ago, for similar reasons: online (HTML), PDF
generation, and printed are all options using it.
I wasn't really part of the decision on it and didn't do any of the
work to use it, so I'm not all that familiar with its use.
Our online version is at http://help.joborder.com if you want to see
it in action. The information is most likely outdated, but at least
you can see how RoboHelp works from the user perspective. It's
actually pretty cool.

I think either FrameMaker or InDesign can do that kind of thing.
