I've modified the "On request" handler in Active4D.a4l, but it does not seem to be taking effect.
Works for me every time, I'm not sure what is going wrong. I know this may sound dumb, but you did uncomment the method, right? Also, if your database is compiled, it may take up to a minute for library changes to take effect.
What happens if you return("foo")? That should cause every request to fail.
