You can apply a graphic to the msgina.dll file to show a company logo (refer OEM modifications) etc. although this will not address your requirement of the extra text field. The biggest issue you would have with writing a custom gina is the ongoing support for it. As it is a common component that is subjected to continuing development you would have to ensure that you match the functional requirements of each modification requirement from MS.
