You can apply a graphic to the msgina.dll file to show a company logo
(refer OEM modifications) etc. although this will not address your
requirement of the extra text field. The biggest issue you would have
with writing a custom gina is the ongoing support for it. As it is a
common component that is subjected to continuing development you would
have to ensure that you match the functional requirements of each
modification requirement from MS. This site will give you some info

the logos windows displays dureing the Ctr-alt_del to start screen
before the logon?

Yup, that's how Novell does it.  They replace MSGINA.DLL with their
custom Client32 one.
