Online defragmentation of the database (ntds.dit) is done automatically.  One thing 
the online defrag doesn't do is to reduce the size of the database.  This can only be 
done with an offline defrag.

Generally, you would only need to run an offline defrag after some major change to 
your AD, e.g. removal of a number of OUs following a company spin-off.
