I want to say it's a known issue, but to tell the truth my brain is going another direction at the moment. I just remember that cached mode and access were a strange combination and just can't recall where I read the details at the moment. If I can find it, I'll post it, but you may want to have a look at the hotfixes/sp's out there for OL2K3 related to cached mode. As well as the known issues for this.
Sorry not to be more helpful, but pretty sure it's your issue.
