I used dsadd to create this user and did not specify a SAM id, so the system
should create it. The user's name is (e.g.,) Frank Burns. I set the upn,
first name, last name, etc. but the system created the SAM id as Frank
Burns, not fburns. 
 
Lesson learned. Is it correct that there is no way to change the SAM id for
an extant user?

Verify the sAMAccountName of the account and that it is indeed user.

It is possible that I have just been looking at this too long.... Why would
it be that a particular user can log in only as [EMAIL PROTECTED] rather
than just "user" and "mycompany" in the domain field?
 
This is a single domain (and single DC) test environment. Other users can
log in with either method.
